At MHA, we improve the health of our community. We are working to promote healthy behaviors and improve access to quality health care for our community.
Who We Are
MHA is a rural health network formed in 2016 by RCMS, CLSD, and Santa Rosa Memorial Hospital who are working together to expand access to healthcare. We’re made up of a Board of Directors and a small staff, including two Community Health Workers, one of which is bi-lingual, a Network Coordinator and an Executive Director.

Our Care Transitions Program
In late 2017 we launched a Care Transitions program that involves having our Community Health Worker reach out those who have been discharged from the hospital and provide support when they come home. For example, our Community Health Worker will make sure you know what medications you’re supposed to take; determine what in-home, transportation, or other assistance you may need; and help you be prepared for your next doctor visit.
A focus on High Blood Pressure…
In 2018 we are providing blood pressure screening events and educational workshops throughout the community every three months. We have hosted “What’s With All the Pressure?” workshops to teach people how to prevent or manage their high blood pressure. Our next round of workshops will be about healthy diet, exercise, and technology tools for home monitoring of blood pressure. We are also facilitating a support network for those in our community who have high blood pressure. This network will provide a source of encouragement and information sharing.
